The Workflow
| Workflow | What to Do |

| Establish baseline | Look back 2–4 weeks and understand the resident’s usual pattern. |
| Review last 7 days | Mark whether each of the 10 care signals changed from that baseline. |
| Connect the pattern | Do not overreact to one isolated item. Look for repeated changes or multiple signals moving together. |
| Clinical review | Record the evidence and let the appropriate clinical team decide whether meaningful decline is present. |
| Care plan + setting check | If meaningful decline is confirmed, compare current needs with the documented care plan and current care setting. |
